What can I request online?

Use our online tool to request things like:

• Medical certificates (Fit Notes or sick notes)
• Referral letters
• Insurance forms
• Travel vaccination forms

What we don’t provide

Please note that certain letters and forms fall outside of our core NHS services. These types of requests often require a more detailed assessment and fall outside the scope of what the NHS will cover. This means that for some of these forms, we may charge a fee to complete. Others we may not be able to complete at all. You can find more details here.

We will aim to complete your request as quickly as we can, although please be aware that some requests can take some time for us to complete.
